Risk Adjustment Redefined

Inferscience is proud to announce the release of an innovative new technology for optimal HCC coding at the point of care in real time. The HCC Assistant was designed to optimize accurate and real time documentation of the patient conditions critical to predicting the risk and future cost of Medicare Advantage patient populations.

There are more than 9,000 ICD-10 codes that map to 70 HCC codes in the Risk Adjustment Model. Providers may not know which HCC code to document in the encounter before the claim is sent which often leads to time consuming retrospective queries to providers and chart mining. The result could lead to suboptimal documentation and lost revenue for the practice or other healthcare organization.

The HCC Assistant parses the patient chart to show all possible HCC diagnosis codes in real time optimizing accurate documentation of the patient visit. Within existing physician workflow, selected diagnosis can simply be added to the assessment in the EMR encounter note with a single click. The HCC Assistant also saves valuable provider time by suggesting care documentation for the diagnosis that can be easily edited and added to the plan section of the chart. HCC Assistant is integrated with multiple EHRs, which allows it to leverage structured and unstructured data in the patient chart.

Inferscience also offers a stand-alone version of the HCC Assistant which will allow users to analyze any digital clinical document for HCC diagnoses by copying and pasting the document into the HCC Assistant. Within seconds, the HCC Assistant displays all possible HCC diagnosis codes and ICD-10 codes which can easily be copied back into the EMR. Providers who are looking at improving their HCC Medicare risk adjustment coding need to take a serious look at HCC Assistant.
